What is Home Health?
Home health care services are provided in your home by skilled professionals. Home health care is available for individuals that need skilled care, are homebound and are under the care of a physician. Your Home Care Team is comprised of skilled professionals which may include Nurses, Therapists, Social Workers and Nurse Aides to provide physician directed care in your home.
What Medicare Pays
Home Health Services are covered by Medicare for eligible beneficiaries. To qualify for home health services with Medicare you must meet the following criteria:
Who Refers to Home Health?
If you have had a recent change in your medical condition or have been recently hospitalized your physician may determine that you would qualify for home care services. Your physician will then make a referral to a qualified home health agency of your choice.
